Enter your First Name, Last Name, and Date of Birth to verify your identity. Click on the “email User ID” box. Your User ID will be sent to your email.

You will receive a temporary security code via SMS (text message) or your preferred authentication method. Enter the code you received via text message and click on the “Submit” button. You will receive a message that a temporary password was sent to your email.
